Opt-Out/Exemption Forms for Statewide Tests

Hillsboro School District hopes to administer state assessments to ALL eligible students. While it is the parent/guardians' right to request opt-out/exemption from state testing for their student(s), we encourage parents to allow their students to test. The information our district and schools obtain from state test results is used to help us improve our teaching.
Students do not "pass/fail" these tests. The tests simply indicate to us where and how we need to improve instruction so that ALL students succeed.
Below please find links to forms for opting out or exempting your student(s) from state assessments. There are two methods by which parents/guardians may request that their student(s) not take state assessments. One is to download, print, complete and sign the form(s) and submit the form(s) to the school that their student(s) attends. The other is to complete the online form indicated below.
