Opt-Out/Exemption Forms for Statewide Assessments and Surveys

Hillsboro School District hopes to administer state assessments to ALL eligible students. While it is the parent/guardians' right to request opt-out/exemption from state testing for their student(s), we encourage parents to allow their students to test. The information our district and schools obtain from state test results is used to help us improve our teaching.
Students do not "pass/fail" these tests. The tests simply indicate to us where and how we need to improve instruction so that ALL students succeed.
Below please find links to forms for opting out or exempting your student(s) from state assessments. You may download, print, complete and sign the form(s) and submit the form(s) to the school that your student(s) attends. Paper forms may also be obtained from the office of your student's school.
Paper Opt-Out/Exemption From State Testing
To Opt-Out/Exempt your student(s) using the downloadable forms, click the appropriate link below. SEED Survey Notice and Participation Form
To decline a student's participation in the SEED Survey using the downloadable forms, click the appropriate link below. Parents/Guardians/Students who wish to exercise their right to not participate in the SEED Survey are asked to complete and sign the form and deliver it to the main office of their student's school.- English Form
