2024-25 Annual Notice and Declining Participation Form for the SEED Survey

The Oregon Department of Education is deeply committed to helping each and every student learn, grow, and thrive. ODE is also committed to providing opportunities for feedback and input to continuously improve education experiences for all students in Oregon. The Student Educational Equity Development (SEED) Survey will help ODE gather information about the educational experiences of students in Oregon.

Online Opt Out Available

Families may also use the online Opt-Out form found in ParentVUE to opt their child out of the SEED Survey. 

New Online Process:

We're excited to offer a new, convenient way to opt your student out of or request an exemption from state testing! You can now complete this process online through your ParentVUE account. 

  • To access the online opt-out/exemption form:

    1. Log in to ParentVUE. 
    2. Select "Online Registration."
    3. Choose "State Assessment Opt-Out or Exemption."
    4. Follow the on-screen instructions.

  • Important Notes about the Online Process:

    • ELPA exemption requests are not available through this online form. Contact your child's case manager for assistance with ELPA exemptions.
    • Only the tests your student is eligible for will be displayed for selection.
