Employee Flu Shot Clinics

HSD and Get a Flu Shot are hosting two outdoor Flu Shot Clinics for employees and their covered dependents:

  • Saturday, Oct. 24, 9 a.m. to 1 p.m. in the Century High School student parking lot
  • Saturday, Oct. 31, 9 a.m. to 1 p.m. in the Liberty High School student parking lot

At press time, there were only a handful of appointments left on the 31st. Check the sign-up sheet for any remaining opportunities. Walk-ins will be accepted at both clinics, but you may have to wait. 


  • Everyone must wear a mask at all times.
  • Maintain at least six feet of distance from others if there is a line.
  • Dress for the weather, but also remember the nurse will need fairly easy access to your upper arm for the injection.
  • Bring your insurance card so that insurance can be billed.
  • If you do not have insurance or you have an insurance carrier that is not on the list, you may pay out of pocket ($30).
  • Dependents must be willing participants and be at least 9 years old.
  • Kaiser members will have to fill out a specific claim form that is only available at the clinic location. Moda members can print and fill out their claim form ahead of time OR fill it out at the clinic. Blank forms will be available.
